What passed before the fixes

The form and mobile layout were not the problem.

The page returned 200 at both sizes, produced no horizontal overflow, exposed one H1 with ordered H2/H3 structure, had no missing image alternatives, no unlabeled visible form controls, and no duplicate IDs. An empty submit was blocked and focus moved to Business name.
